Please note that due to COVI-19 we have reduced office hours from 8:30 a.m. to 5:00 p.m. Monday through Wednesday. We are taking all CDC precautions for extra cleaning and social distancing to ensure the safety and welfare of our patients. As many skin conditions have similar symptoms and appearances, an accurate diagnosis is usually possible only with an office visit. We maintain daily work-in appointments for urgent patient needs; however, priority will be given to patients with regularly scheduled appointments.

We make every effort to stay on time; however, there are times when the physician may be detained. Please notify us at least 24 hours in advance if you are unable to keep your appointment. This enables us to see emergency patients on a responsive basis. If you will be unavoidably late, please call us.

**Please note – this form is for requesting an appointment – it is not a guarantee of an appointment. Once we receive your request, we will contact you to schedule an appointment.**
